The Requests Form module allows you to create any number of form templates.
Any employee who wants to submit the appropriate form logs into their account, finds the appropriate request, fills it out, and submits it online. The request goes to the employee defined in the approval path. The employee who submitted the request can view the status of the request at any time and knows the entire approval path. They also have access to an archive of all requests they have submitted, along with information on whether they were accepted, rejected, or canceled.
